Choosing to Get Blepharoplasty

The eyes are often one of the most notable facial features, capable of expressing a range of emotions without needing to say a word. Yet sometimes, the eyes can give off a tired, bored, or aged appearance, even when the person does not intend for that to be the case. This is often the result of sagging eyelids, puffiness, or under-eye bags. 

Blepharoplasty can be a life-changing option for those struggling with excess skin and fat in the eyelids. This surgical procedure tightens the eyelids and repositions underlying tissues to create a smooth, rejuvenated look. 

Where Are Blepharoplasty Incisions Made? 

Each blepharoplasty procedure is tailored to address the patient’s concerns and create their desired look. The locations of the blepharoplasty incisions will depend on the type of eyelid surgery the patient has selected. Blepharoplasty can be performed on the upper eyelids, lower eyelids, or a combination of the two. 

Incisions will be made on the upper or lower eyelids based on the blepharoplasty option you choose. Below, we’ve broken down the main incision options so you can get an idea of what they look like. 

  • Upper Blepharoplasty: The incisions are created on the upper eyelid’s natural crease. This allows for hanging eyelid skin to be removed, eliminating a droopy look. The remaining eyelid skin is tightened for a smooth and refreshed aesthetic. Upper blepharoplasty scars are typically unnoticeable after 4 to 6 weeks.
  • Lower Blepharoplasty: For lower eyelid surgery, incisions are made in the lower lash line. This hidden incision allows for the underlying tissues to be adjusted and for excess fat to be removed, creating a smooth surface in the under-eye region. Lower blepharoplasty scars typically fade and are difficult to find after 6 to 12 weeks. 
  • Quad Blepharoplasty (Combination of Upper and Lower): For patients who want to address aging signs in both the upper and lower eyelids, quad blepharoplasty can be a great fit. The incisions are created in both the upper and lower eyelids, as described above. 

What Do Blepharoplasty Scars Look Like? 

Blepharoplasty scars will form in the locations where the incisions were created. The scars will evolve over the blepharoplasty recovery period and will continue to fade months and years after the surgery. Initially, after surgery, the incision sites will be red and swollen. Some bruising may develop around the area, which will fade over the next several weeks. 

One to three months after blepharoplasty, the swelling and bruising will mostly subside, and the scars will appear red or pink. During this time, the raised texture of the scars will begin to flatten and appear less noticeable.

Once three to six months have passed, the blepharoplasty scars will fade and start to blend in with the skin. Redness is typically gone at this point. In addition, the placement of the blepharoplasty incisions makes faded scars hard to detect, as they are hidden along the natural eyelid or lash line. 

One year after surgery, the scars are considered to be fully mature. At this time, they will be flattened and only faintly apparent, as they will blend in with the eyelid’s natural creases.
